By completing and submitting this registration form, I acknowledge, understand, and agree to the following terms on behalf of myself and/or the participating child: 1. Medical Treatment Authorization In the event of accident, illness, or injury, I authorize the event organizers, their appointed staff, and qualified medical personnel to provide or obtain necessary first aid or medical treatment. This includes arranging ambulance transport if required, when deemed necessary for the safety and wellbeing of the participant. I understand that all medical, hospital, and related costs incurred are my sole responsibility, and I agree to reimburse any expenses borne on my behalf by the organizers. 2. Acknowledgment of Risk & Waiver of Liability I acknowledge that participation in football activities inherently involves physical exertion and potential risk of injury, illness, or property loss. I voluntarily assume full responsibility for these risks, recognizing that while the organizers take all reasonable precautions, injuries may still occur. I hereby release, indemnify, and hold harmless the event organizers, their officers, staff, volunteers, coaches, venue operators, and affiliates from any and all liability, claims, or damages arising from participation in this event, except in cases resulting directly from their proven gross negligence or wilful misconduct. 3. Media & Marketing Consent I grant permission for photographs, video recordings, or digital media featuring myself/my child taken during the event to be used by the organizers for legitimate promotional, marketing, and educational purposes, including but not limited to social media, websites, and printed materials. I understand that these materials may be publicly distributed and that no financial or other compensation will be provided for such use. If I do not consent to media use, I will notify the organizers in writing prior to the commencement of the event. 4. Declaration of Accuracy I declare that all information supplied within this registration form is true, complete, and accurate to the best of my knowledge. I understand that providing false or misleading information may result in exclusion from participation without refund.
